"Free" means different things to different vendors. Here's what each option actually gives you before you put a year of patient records into it.
Every clinic software vendor in India offers something called "free" — a trial, a free tier, an open-source download, or a genuinely free-forever plan. Those are four very different promises, and the difference matters more once your patient history is inside the software than it does on day one. This guide sorts through the options doctors actually consider, based on what each vendor currently publishes about its own pricing.
The four kinds of "free"
Before comparing specific tools, it helps to know which bucket a "free" claim falls into:
- Open-source, self-hosted. The software itself costs nothing and stays free forever, but you (or someone you hire) handle installation, hosting and maintenance yourself.
- Capped free tier. A real free plan, but scoped down — a patient limit, no reception login, or missing features — designed to convert you to a paid plan once your clinic grows into it.
- Free trial. Full features for a limited time, then a paid subscription or a custom quote. Not really "free" software — a free evaluation period.
- Free forever, for a defined use case. A genuinely free plan with no time limit and no artificial cap, usually because the vendor makes money on optional add-ons instead of the core software.
OpenEMR — free and open-source
OpenEMR is a mature, GPL-licensed open-source EMR with a global user base. It's genuinely free to use, but you (or someone you hire) are responsible for hosting, updates and India-specific customisation. It's a strong option for a clinic with technical capacity; less so for a solo doctor who wants to sign up and start seeing patients the same day.
MyOPD — capped free tier
MyOPD's free trial includes limited patient records. According to MyOPD's official pricing page, its Clinic version is listed at ₹8,000/year or ₹18,700 as a one-time purchase, while updates, cloud backup and support may carry maintenance charges from Year 2. It is a legitimate option with regional-language prescription support; confirm the current limits and total cost before choosing.
HealthPlix — paid Pro and Elite plans
HealthPlix's official pricing page currently lists Pro at ₹11,999/year and Elite at ₹17,999/year. Both include broad clinic and specialty workflows; confirm current offers directly before buying.
Doccure — configurable one-time-purchase option
Doccure's official solo-practitioner page describes a one-time purchase with optional add-ons, but does not publish the amount. It offers a broader configurable platform, so ask for the exact package, deployment model, support terms and total price in writing.
SoapNote — free forever for one doctor

Whichever tool you pick, ask these before moving a year of patient records: What exactly is included in the free tier — and what's the first thing that isn't? What does the vendor charge once you outgrow it? Do you keep your data if you stop paying? Does it work when your internet doesn't? Get the answers in writing, not just from a sales call.
The bottom line
If you want zero licence cost and can manage hosting, OpenEMR is the strongest open-source option. If you want an India-focused product with no setup burden and a free solo-doctor core, that's the gap SoapNote is built to fill.
