Offline vs. cloud EMR: what happens if you stop paying?

It's a question worth asking before you sign up, not after your renewal notice arrives.

Most clinic software today is "cloud EMR": your patient records live on the vendor's servers, and you access them through a browser or app for as long as your subscription is active. It's a genuinely good model for a lot of practices — automatic updates, access from anywhere, no server to maintain. But it comes with a dependency that's easy to overlook when you're comparing feature lists: your ability to see your own patients' history is tied to a recurring payment.

What "cloud-dependent" actually means day to day

For most cloud EMRs, three things are true by design: you need an internet connection to see a patient's chart, your records live on infrastructure you don't control, and continued access is conditional on an active subscription. None of that is inherently a problem — it's the trade-off cloud software makes for convenience. The part worth thinking through in advance is what happens at the edges: a patchy internet day in the clinic, a lapsed renewal during a busy month, or a vendor changing its pricing or shutting down.

Questions worth asking before you commit

  • If I stop paying, do I keep read access to past records, or does access stop entirely? Vendor policies vary widely here, and it's rarely the headline feature on a pricing page.
  • Can I export everything — including scanned documents and images — in a format another system can read? A CSV of patient names isn't the same as a portable clinical record.
  • What happens during an internet outage? Some cloud EMRs cache recent records locally for offline viewing; many don't.
  • Who has access to my patient data, and is it ever used beyond running my clinic? Worth reading the actual privacy policy, not just the marketing page.

The offline-first alternative

The other model — less common, but not new — is offline-first software: the application and your patient records live on your own clinic computer, and the internet is optional rather than required for daily work. The trade-off is the flip side of cloud convenience — no automatic access from an arbitrary device unless you set up your own sync, and you're relying on your own hardware rather than a vendor's infrastructure for day-to-day uptime. Neither model is universally "right"

A busy multi-location clinic that needs real-time access from five devices has good reasons to lean cloud. A solo practice that wants certainty that its records will still be there — and still be readable — a decade from now, regardless of what happens to the vendor, has good reasons to lean offline-first. The mistake is not asking the question at all before a year of patient history is already inside one system or the other.
